In Person Requests
Requests for fire incident reports can be requested in person during the following hours:
Monday - Friday
9 - 11 a.m.
1:30 - 4 p.m.
Requests can be made in person at the Rescue Division office on the 3rd floor of the Fire Administration Building.
Fees
The fee for a fire report is $1.00 per certified page and $0.15 per page per copy (amount of pages per report will vary). Cash, check, or all major credit cards (except American Express) are accepted.

Mail your request and payment to:

Please include a self-addressed, stamped envelope where you wish to receive the records.
Fees
When requesting a report by mail, please call to find out how many pages, and attach a check payable to “Hialeah Fire Department” for the correct amount. The fee must be received before the report will be sent.
Records in response to subpoenas are returned by certified mail. Please add an additional $7.50 for the cost of this service.
