How to Request a Fire Incident Report

Fire Incident Reports may be requested in person or by mail.

IN PERSON: Monday thru Friday from 9:00am to 11:00am and then again from 1:30pm to 4:00pm

  1. The fee for a Fire Report is $1.00 per certified page (amount of pages per report will vary). Cash, check, or all major credit cards except AMEX are accepted.


BY MAIL: 
Mail your request and payment to:

  1. The fee for a Fire Report is $1.00 per certified page (amount of pages per report will vary). Please call to find out how many pages, and attach a check payable to “Hialeah Fire Department” for the correct amount. The fee must be received before the report will be sent.
  2. Records in response to subpoenas are returned by certified mail. Please add an additional $6.50 for the cost of this service.
  3. Please include a self-addressed, stamped envelope where you wish to receive the record(s).
