Requests by Attorneys / Law Firms

Completing the Form

Your request must be made in writing on the Medical Authorization Form (PDF) provided by the City of Hialeah Fire Department. Please complete the form by providing all the information requested. Please note the following:

  • If you are represented by an attorney, identify on the attorney or firm to whom the reports will be released to on the form. The request must be accompanied by a letter of representation on your attorney’s letterhead.
  • The form must be signed in the presence of a Notary Public.

Fees

The fee for a Medical rescue report is $1.00 per certified page and $0.15 per page per copy. The amount of pages per report will vary. 

Please call to find out how many pages, and attach a check payable to “Hialeah Fire Department” for the correct amount. The fee must be received before the report will be sent.

Records in response to subpoenas are returned by certified mail. Please add an additional $7.50 for the cost of this service.

Submitting Your Request

The original, notarized request form, payment, and any accompanying documents should be mailed to:

Please include a self-addressed, stamped envelope where you wish to receive the records.

A photocopy of the request form will not be accepted. We will not process any requests received by fax or email.

In Person Requests

Requests for rescue incident reports can be made in person during the following hours:
Monday - Friday
9 - 11 a.m.
1:30 - 4 p.m.

Requests can be made in person at the Rescue Division office on the 3rd floor of the Fire Administration Building.

Required Documents

You must bring valid picture identification with you. 

If you are not the patient listed on the report, you must bring an original request Medical Authorication Form (PDF) provided by the Hialeah Fire Department. Please complete the form by providing all the information requested. Please note the following:

  • The form must be signed in the presence of a Notary Public. 
  • The person picking up the reports must be listed on the form and have valid picture identification.
  • A photocopy of the request form will not be accepted.

Fees

The fee for a fire report is $1.00 per certified page and $0.15 per page per copy. The amount of pages per report will vary. Cash, check, or all major credit cards (except American Express) are accepted.
